Thank you for your interest in Michael Lienhard Psychiatry – Dr. Lienhard and/or a staff member will be reaching out by phone in the next 24-72 hours to discuss your case further.
If you and Dr. Lienhard decide to schedule an appointment, please think about how you would like to go about completing the initial intake paperwork. There are four options:
1. Electronic Medical Record (EMR) – Dr. Lienhard uses the Luminello EMR, which includes an online client portal for secure HIPAA-complaint messaging, appointment notifications and reminders, and management of information electronically. If requested, you will receive an email invitation from Luminello to create an account of your own, after which you can download and submit the intake forms on the convenient and user-friendly portal.
2. Email – If requested, you will receive a blank intake packet to your email, which can be completed on your electronic device or printed and filled out by hand. When the forms are complete, a printed physical copy should be brought to the first appointment. Alternatively, you can send the completed paperwork via fax to (609) 875-1310. You should NOT email the completed forms back via regular email – unsecured and unencrypted messages (and any attachments) can be read, and potentially copied and forwarded, by anyone. As there are risks associated with unsecured communication, sharing personal health information (PHI) over email is not recommended.
3. Mail – If requested, you will receive a blank intake packet via mail to your desired physical address, which can be filled out by hand. When the forms are complete, the physical copies should be brought to the first appointment. Alternatively, you can send the completed paperwork via fax to (609) 875-1310. As explained above, completed forms should NOT be sent back via email given the risks associated with sharing personal health information (PHI) over unsecured and unencrypted messaging.
4. In-Person – If requested, you can complete the intake paperwork on-site at Michael Lienhard Psychiatry, 155 North Dean Street, Suite 3E, Englewood, New Jersey 07631. Recommended arrival time is 30-minutes prior to your first scheduled appointment to allow adequate time for completion of intake forms.
In addition to completed intake forms, you must also bring an appropriate photo ID and any accompanying legal paperwork (e.g. guardianship documents, family court documents, etc.) that may become relevant during evaluation and treatment.
